At HP Homecare, we understand the importance of continuity of care for our clients.

As a leading provider of home care services, we have been serving individuals in need of private nursing and home care across London for over 20 years. Building strong professional relationships, in the comfort of a client’s own home, is at the centre of our practice.

High-quality home care is about personalising care to meet specific needs. Each client has unique needs and preferences, and it is our responsibility to adopt a multi-disciplinary approach in order to understand and fulfil these needs. This approach demonstrates our commitment to providing tailored care services for each and every client we support.

Continuity of care is established through trust, a strong professional relationship, and consistency in a client’s care team. By focusing on these three elements, our care teams get to know their clients, understand their needs and preferences, and build a caring relationship based on confidence and compassion.

Our Director of Homecare, Michele Jogee, and her team of professionally trained nurses and carers work closely with our clients to co-produce bespoke care plans that meet a client’s specific requirements. This ensures that our clients receive care that is designed with their wellbeing at the centre, and promotes autonomy, dignity and ownership over their own home care journey.

We also recognise that continuity of care is crucial not only for our clients but also for their families. We collaborate with our clients’ families to involve them in the care planning process and keep them informed about their loved one’s care. This helps to foster trust and understanding between our care team and our clients’ families, which is vital for providing holistic, effective care.

Our care team consists of carefully-selected nurses and carers who we would trust to care for our own loved ones. With a minimum requirement of two years’ home care experience, our teams are as proficient in administering medications and changing dressings as they are in daily care routines. We believe that this level of experience and proficiency is crucial for building strong relationships with our clients.

In addition to this, we emphasise that continuity of care also means that a client receives the same level of care from each team member. We ensure that all of our nurses and carers are trained to the highest standards, and are equipped with the knowledge and skills needed to provide the best possible care.

At HP Homecare, we value the importance of continuity of care in building strong relationships with our clients. By personalising care plans to meet the specific needs of each individual, and ensuring consistency of the care provided, we strive to deliver the best possible care for our clients.
